Blount disease is a challenge to the pediatric orthopaedic surgeon due to the defect on the medial aspect of proximal tibial physis, with varus, procurvatum and internal rotation of the lower limb. Treatment should include surveillance of growing process, focusing on the correct alignment and limb equality. Type 6 of Langeskiold is the most severe form. A girl with this presentation is reported in this study with the serial surgeries needed to achieve this goal. She has undergone oblique acute osteotomy for correction of varus and rotation, medial plateau elevation, medial distal femur epiphysiodesis, and two metaphyseal osteotomies with circular fixator application, the last one with an hexapodal frame.
