The authors report a case of Primary Hypertrophic Osteoarthropathy - complete type - in a 26-year-old male with severe manifestation of periosteal synovitis, and pronounced knee effusion bilaterally, without orthopedic treatment for more than 10 years. Bilateral open synovectomy procedure is described, with reconstruction of the knee extensor mechanism, unpublished procedure in the treatment of this pathology. Results of the surgical procedure are described, with 6 months follow-up.
