Abstract

Humeral capitellum fractures are infrequent injuries of the upper limbs, and their suspicion as a differential diagnosis from elbow fractures is important, as well as quality radiographic images to facilitate identification. These fractures are intra-articular and result from a shearing force of the radial head on the capitellum, and may present associated injuries such as ipsilateral fracture of the radial head, ipsilateral fracture of the distal radius, and ligament injuries of the elbow. Joint stiffness is the main complication, and early mobility in the postoperative period is encouraged to avoid this. Among the treatment options is fracture reduction and osteosynthesis, which can be performed through an open or arthroscopic approach. Arthroscopic reduction and synthesis have several advantages: lower infection rates, direct visualization of the joint surface and its reduction, in addition to less tissue violation to access the joint (as it does not require muscle or ligament disinsertions for the surgical approach). A challenge for this type of fracture is that due to its low incidence, there are few publications on this subject, making it difficult to standardize the treatment.
