Fractures of distal humerus in adults are a challenge considering surgical treatment. Fixation with medial and lateral locked plates has been the treatment of choice for joint reconstruction. In ulnar nerve lesions the use of a graft and neurotization of the anterior interosseous nerve is an alternative. This report addresses surgical treatment of a fracture of the distal humerus by a firearm projectile using blocked plates and reconstruction of the ulnar nerve with sural nerve graft and neurotization of the anterior interosseous nerve.
