Congenital deficiency of the femur is characterized by complex bone and soft tissue changes, and its treatment involves bone reconstruction and limb lengthening. This reported patient had Paley type 1A, without requiring complex joint realignments, and femoral lengthening with a monoplanar external fixator was proposed. Due to intense physical therapy to maintain knee mobility, there was flexion of the regenerate. Correction with extensor osteotomy using a minimally invasive technique, and stabilization with blocked intramedullary nail was proposed.
